Service-Connection Types
Overview
The VA recognizes four distinct categories of service-connection, each establishing the relationship between military service and a veteran's disability.

Key Distinction
There are NO differences in how these types of service-connection conditions are rated. All service-connected conditions are rated using the same rating schedule based on severity.
However, secondary conditions cannot have effective dates earlier than the service-connected condition causing them.
Primary (Direct) Service-Connection
This encompasses disabilities resulting from military service or occurring during active duty, including those caused by toxic exposures.
Requirements for Direct Service-Connection
- In-service event, injury, or illness - documented occurrence during military service
- Current disability - present diagnosis or functional impairment
- Nexus - medical link between the in-service event and current disability
Examples
- Hearing loss from military noise exposure
- Back injury from military training
- Respiratory condition from burn pit exposure
- PTSD from combat experiences
Note: Conditions originating during service that create secondary disabilities are classified as primary by the VA.
Secondary Service-Connection
This involves disabilities caused by already service-connected conditions. Secondary conditions can themselves generate additional disabilities (tertiary conditions), and each receives independent ratings.
Common Secondary Relationships

Key Points
- Primary conditions rated at 0% can still have secondary conditions
- Secondary conditions can rate higher than their primary condition
- There is no limit to the number of secondary conditions
- Medications causing side effects can create secondary conditions
Presumptive Service-Connection
Certain veteran groups may qualify for automatic service-connection of specific disabilities without proving direct causation, provided eligibility criteria are satisfied.
Presumptive Categories
- Chronic conditions within one year of separation

Aggravation Service-Connection
This applies when military service or an existing service-connected condition worsened a pre-existing disability.
Two Types of Aggravation
- Service aggravation - pre-existing condition made worse by military service
- Secondary aggravation - non-service-connected condition made worse by a service-connected condition
Baseline Requirement
The VA establishes a "baseline" level of disability before aggravation. Only the increase above the baseline is compensated.
Example
- Veteran enters service with 10% level back pain
- Military service aggravates it to 30% level
- VA compensates for the 20% increase (difference between pre-service and current severity)
