Special Monthly Compensation (SMC) - Complete Guide
Overview
SMC provides additional compensation to veterans with service-connected disabilities beyond the standard 100% disability rating. "In certain circumstances the VA will pay compensation to Veterans with certain needs or disabilities at a rate higher than 100%."
Chief's Take: SMC is the VA's secret menu. It's extra compensation for specific severe situations that go beyond the regular rating schedule. Most veterans don't know it exists, and the VA isn't exactly advertising it. Study the chart. You might be leaving money on the table.
Key General Points
- SMC is typically awarded automatically when eligibility criteria are met
- Exception: SMC K, L, R, T, and Housebound compensation generally require formal application
- Most SMC levels are not paid alongside the veteran's base disability rate
- Only SMC K and obsolete SMC Q are paid in addition to disability compensation
- Higher SMC levels may be discontinued during government-sponsored hospitalization
SMC Levels & Eligibility
SMC K (Amputation/Loss of Use)
Eligibility: Awarded for loss or loss of use of specific body parts: - Complete joint immobility or limb paralysis - Loss of sight in one eye (light perception only) - Loss or loss of use of hand or foot - Loss or loss of use of reproductive organs (erectile dysfunction, FSAD, etc.) - Complete loss/loss of use of both buttocks - Deafness of both ears - Complete inability to speak (organic aphonia) - Loss of 25%+ breast tissue from mastectomy or radiation (females only)
Important Notes: - Elective vasectomy or hysterectomy disqualifies applicants unless service-connected for reproductive organ dysfunction - Up to three SMC K awards possible per veteran - One award per limb; reproductive organ loss counts as single award - Paid on top of disability compensation (except when SMC O, Q, R, T applies)
SMC L (Aid and Attendance)
Eligibility - Meets ONE criterion: - Anatomical loss or loss of use of both feet - Anatomical loss or loss of use of one hand and one foot - Blindness in both eyes - Permanently bedridden status - Daily assistance needed for basic life activities - Disability requiring another person to prevent self-harm or harm to others
Policy Requirements: - Service-connected disabilities qualifying for aid/attendance must typically combine to 100% or have individual 100% rating - If not met, VA Central Office advisory opinion required - Consider also applying for Program of Comprehensive Assistance for Family Caregivers (PCAFC)
SMC L ½: More severe version with anatomical losses extending to elbow and/or knee
SMC M (Severe Multiple Losses)
Eligibility - Meets ONE criterion: - Anatomical loss or loss of use of both hands - Anatomical loss or loss of use of both legs at knee region - Anatomical loss or loss of use of one arm at elbow with one leg at knee - Blindness in both eyes with light perception only - Blindness in both eyes requiring regular aid/attendance
SMC N (Extreme Bilateral Loss)
Eligibility - Meets ONE criterion: - Anatomical loss or loss of use of both arms at elbow region - Anatomical loss of both legs near hip preventing prosthetic use - Anatomical loss of one arm near shoulder and one leg near hip (both preventing prosthetic use) - Anatomical loss of both eyes or bilateral blindness including light perception loss
SMC O (Most Severe Conditions)
Eligibility - Meets ONE criterion: - Anatomical loss of both arms near shoulder (preventing prosthetic use) - Bilateral deafness rated 60%+ with service-connected blindness (20/200 or worse) - Complete deafness in one ear OR bilateral deafness rated 40%+ with bilateral blindness including light perception loss - Paraplegia with bowel/bladder incontinence - Helplessness from combined anatomical losses, deafness, blindness, or multiple severe injuries
SMC P (Half-Step Increases)
Purpose: Increases SMC rating by one half-step when eligibility criteria met
Eligibility - Meets ONE criterion: - Leg loss below knee on one side, above knee on other side - Leg loss below knee plus arm loss above elbow - Leg loss above knee plus hand loss - Blindness in both eyes meeting SMC L, M, or N requirements
Limitation: Does not apply to SMC K, S, O, Q, R, or T
SMC S (Housebound)
For veterans with a single 100% disability who are housebound due to that or other service-connected conditions.
SMC R
Two levels (R1 and R2) for veterans requiring higher levels of aid and attendance.
SMC T
For traumatic brain injury residuals requiring assistance with activities of daily living.
Application Process
Automatic Award: SMC K, L, M, N, O, P generally awarded automatically when evidence supports eligibility
Required Filing: - SMC K requires standard VA claim submission - Aid and Attendance (SMC L) and Housebound compensation require specific application procedures - Application for SMC is NOT automatically considered a claim for increase, though VA may treat it as such
Important Pyramiding Rules
- Veterans cannot receive same SMC K award twice for same limb
- Single reproductive organ award covers all related conditions
- When higher SMC granted based on loss of use conditions, SMC K terminates (except during government hospitalization)
- Legal Exception: Veterans with 3-limb loss can receive one SMC K alongside upper-level SMC
Appeals & Disputes
For potentially incorrect SMC levels (especially upper levels L-T), veterans should file a Higher Level Review due to complexity of pyramiding rules.
