| 30% | Motion limited midway between side and shoulder (~45°) (Major), 20% (Minor) |
| 20% | Motion limited to shoulder level (90°) - same rating for Major and Minor |
Important Notes
Higher ratings for dominant (Major) arm. Shoulder level = 90° = arm horizontal. If pain at 90° per DeLuca/Mitchell, functional range capped at 90° = 20% rating. Per § 4.59, painful motion = minimum compensable rating.
